This technique of examination is used to examine the size, shape, and appearance otherwise of the optic nerve. The optic nerve usually appears pink whereas in glaucoma it appears as having a larger central zone - this is known as cupping. |

Gonioscopy is used to check if there is a narrow angle - and to evaluate the risk of developing angle closure glaucoma. Over 80% of patients have open angle glaucoma. Angle closure is commoner in certain races, moreso in those of Asian ancestry. |

The new drug memantine with potential use in glaucoma, has failed to demonstrate a significant change in the primary outcome measure with treatment (confirmed visual field loss), however an animal model has demonstrated an effect in mice: researchers in Japan have recently noted that glaucoma prone mice deficient in small molecule transporters (GLAST & EAAC1) are significantly protected in terms of ganglion cell loss in the laboratory (3% versus 20% loss).

The Canadian Institute for the Blind (CNIB) released results from a large longitudinal study of 258 patients, demonstrating that gender plays an important role. Women were twice as likely as men to develop permanent visual field loss. The new risk factor identified in this study was that anticardiolipin antibodies are associated with progression. |

Selective laser trabeculoplasty (SLT) works by selective thermolysis. An ultrashort laser pulse prevents "collateral damage" associated with ALT. Both ALT and SLT are believed to increase drainage throught the trabecular meshwork by releasing chemotactic factors such as interleukin 1a, interleukin 1b, and TNF. There is some evidence that SLT works effectively in pseudophakic eyes. |

SLT laser is available as an option for use instead of a second line drop. In some studies show a very significant drop in IOP, an average of 7.7 mmHg after SLT in one study (1). (1) Melamed et al, Arch Ophthalmol 2003: 121 957-960. |
